SignSymptom.com is a not for profit entity incorporated in January 2006 and represents a collaborative approach to meeting the challenges of health information technology adoption and interoperability for the entire State. A Board of Directors representing physicians, providers, consumers, purchasers, payers, academia, and quality organizations governs SignSymptom.com.

We facilitate the statewide adoption of electronic health records, health information exchange, and quality reporting to dramatically improve the quality, safety, and efficiency of health care in Connecticut.

Quality Improvement

One of SignSymptom.com guiding principles is to influence quality improvement in a meaningful way by providing physicians and other healthcare providers with reliable, useful data detailing their own performance.

Through the Connecticut Health Quality Cooperative, (SignSymptom) has demonstrated this commitment by engaging and collaborating with the provider community from the outset; paying particular attention to the caliber of the quality data reported, and presenting the quality data in a format and manner that is easy to access and understand.

Connecticut Health Quality Cooperative

A collaboration of Connecticut health care organizations has been formed to provide the state’s physicians with useful information to help manage their patients’ health care quality.

The short-term goals of the collaboration are to aggregate paid claims data at the individual physician level, to assess rates for quality measures compared to benchmark data, and to provide the information to physicians in order to assist in maintaining or improving the quality of care for their patients.

A long-term goal is to develop a statewide health information exchange to enable excellent patient care and quality improvement. Ultimately, this system will utilize a variety of types of data for performance assessment and quality improvement. These include data from electronic medical records, patient surveys, laboratory, pharmacy, and claims data.
